I use dental dams and condoms for all oral; it's one of the things that keeps me from being a $1000 an hour type of escort, who generally doesn't require protection for oral. I'm simply not willing to take the risk.
Not many guys insist on not using a condom for oral, probably because no condom = no oral. Too bad, so sad for them. If a guy can't before my jaw tires out, I'll suggest that we switch to intercourse or he lets me give him a handjob. If his time runs out, I'll give him an extra five minutes or so if he's been nice, then tell him that he should book more time when he comes to see me next.
